Bishop Connolly High School’s Student/Parent Handbook provides families with important information and policies related to academics, athletics and student life. Students and families must complete and submit the 2022-2023 handbook contract and the following mandatory forms.

Mandatory School Forms

Please download and complete the following mandatory school forms and submit to BCHS Administrative Assistant Sandra Furtado at sfurtado@bchs.dfrcs.org. Completed forms may also be returned to the Main Office.

Mandatory Health Forms

Read the following letter from BCHS school nurse Kimberly Jerome, RN, for important information about your student’s health needs.

Please download and complete the following health forms and submit to Mrs. Jerome at kjerome@bchs.dfrcs.org. Completed forms may also be returned to the Main Office.

Please note: Students planning to participate in sports must submit their most recent physical exam to the school nurse in order to take part in any practices/games. New incoming students must also submit their most recent physical exam to the school nurse.

Please download and complete the following mandatory athletic forms and submit to Athletic Director Cesar Goncalves at cgoncalves@bchs.dfrcs.org. Completed forms may also be returned to the Main Office.

Please note: Students planning to participate in sports must submit their most recent physical exam to the school nurse in order to take part in any practices/games.
